Population Medicine Programme


GEMS constantly strives to find innovative ways to provide healthcare to all members, and one of these initiatives is the GEMS Population Medicine Programme.

The GEMS Population Medicine Programme is a value-based care programme designed to help vulnerable populations and those in under-served communities by providing support network that can help address health risks and promote wellness.

The programme is based on the concept of a multidisciplinary team (MDT) which is typically led by a specialist physician, and includes family practitioner(s), nurses (as care coordinators), and allied health providers. This helps to ensure that holistic care, based on evidence-based protocols can be provided to our members.

The focus of the programme is to manage the coordination of care of members who are nearer to where the MDT is located. This is important for beneficiaries with multiple comorbidities, or those with at risk of complications and requiring admission to hospital. 

Depending on your location, and if you meet certain criteria, GEMS may contact you to advise that you are allocated to the nearest MDT, and you will be given the opportunity to enroll on the programme. Please note that a consent is required for your information to be shared with the all the providers that are part of the MDT.
